• A statement, under penalty of perjury, that the information in your notice is accurate and that you are authorized to act on behalf of the copyright owner.
• Your physical or electronic signature.

How to Send Your Notice

Counter Notification

If you believe material was removed or access was disabled by mistake or misidentification, you may submit a counter notice that includes:

• Your full name, postal address, phone number, and email address.
• Identification of the material that has been removed or to which access has been disabled, and the location where it appeared before removal.
• A statement, under penalty of perjury, that you have a good-faith belief the material was removed or disabled as a result of mistake or misidentification.
• Your consent to the jurisdiction of the courts in your area, and that you will accept service of process from the person who provided the original DMCA notice or their agent.
• Your physical or electronic signature.

Repeat Infringers

MP3YT reserves the right to disable access for users who are repeat infringers, and to remove content upon receiving valid and complete DMCA notices.
